Step One.

Step Two.

Download the Print/Electronic Reserve Form [ PDF 43K ]

Step Three.

Print out the completed form using your Web browser by going to the FILE menu and selecting PRINT.

Step Four.

Submit the completed form to Sinclair Library either:

• In person at the Wong Audiovisual Center (3rd floor) during normal business hours
